The pilot who survived a crash into the East River Sunday night that killed his five passengers is from CT, according to the New York Daily News.

As for what caused the helicopter to go down, a source familiar with the investigation says the pilot told police he observed a "tether from a front passenger's harness wrapped around the fuel shut off lever, which had accidentally cut off fuel supply to the engine resulting in engine failure".

Liberty Helicopters said in a statement it was "focused on supporting the families affected by this tragic accident", adding that it was co-operating with investigators.

"That's something we're looking into", Gunther said. These made it hard for them to escape the stricken aircraft.

McDaniel was a firefighter in Dallas who'd gone to high school with Cadigan and was in NY visiting him over the weekend, NBC Dallas-Fort Worth reported.

A bystander caught the incident on video, which shows the helicopter crashing into the water and submerging into the river as the blades keep spinning. As Cadigan grins widely into the camera, he angles his phone to show McDaniel sitting just behind him.

The helicopter was being chartered for a photo shoot when it went down near Gracie Mansion, the mayoral residence.

NTSB spokeswoman Bella Dinh-Zarr said at a press conference this afternoon that the crash occurred at 7:08 p.m. ET Sunday and her team "will be looking at other sister helicopters and see if this helicopter's harnesses were functioning and if the passengers were using them".
